STEP ONE: FILL OUT ONLINE REGISTRATION FORM
  1. CLICK HERE TO DOWNLOAD FORM IN WORD FORMAT.
  2. FILL OUT INFORMATION AND SAVE THE DOCUMENT.
  3. EMAIL FORM TO ALLTHATJAZZ@USSAAC.ORG

STEP TWO: CLICK HERE TO SEND SECURE PAYMENT ONLINE

STEP THREE: USSAAC has arranged another block of rooms with the Astor Crowne Plaza Hotel, right across the street from the Hotel Chateau Sonesta. The are honoring USSAAC's All That Jazz conference rate of $139. To reserve a room, call 888-696-4806

Call-in registration to 831-649-3050 (with credit card information)

 Mail the registration form with your check to USSAAC, c/o One Surf Way, #237, Monterey, CA 93940. Make checks payable to USSAAC: All That Jazz.  We accept all major credit cards and school purchase orders (PO).
